Once you are logged into the Centology Portal, please select the option for 'Forms' from the navigation bar on the left-hand side of the screen:
Once you have accessed the Forms page, you will be able to see any forms that have been pre-completed for you to approve and sign. Click on the pencil icon to edit and sign the relevant form(s):
Clicking on the pencil icon will give you edit access to the form. You can navigate through the forms either using the numbers on top of the form or via the next button at the bottom of the form to individually go through each page:
The colour of the page numbers at the top of the forms indicates whether the client needs to input information. If it's red, the client needs to provide information; if it's blue, no further details need to be filled out for that page.
After filling out the required fields on the forms, you can sign the form either by typing in the signature box or by drawing your signature:
After signing the form, the primary applicant's signature date will be auto-filled with the current date, and you will be able to submit the form via the 'Submit' button:
You will receive a prompt asking, 'Are you sure you want to submit this form?' Once you select 'Yes' the form will be submitted and cannot be altered by the client or Adviser Administrators:
The form is then reviewed by the Adviser Administrators and is either approved or rejected. Please note that only Administrators and Adviser Administrators can approve the forms.
Once the form has been approved, you would need to download the form to see the outputted direct debit mandate (DDM). To download the form, click on the eye icon of the form:
This will take you to the read only version of the form. You would need to click on the the 'Download' button of the form and the DDM should be available to view within the downloaded application form:
